I hope Cas sticks to the foreplay and doesn't go too fast. He's probably eager after a year in purgatory with no chest to fist. I'm sure there will be a frantic first round followed by a much slower and more telling second go at it.

That's all right, Sam likes it when it hurts a little. Or a lot. Castiel's fist inside him so quickly would be overwhelming and yet oh so satisfying.

The first time, Castiel's in too much of a hurry to even tie Sam up, and he just pins him to the floor of the van with brute strength; because there's no belt for Sam to bite on, Castiel just puts his hand over Sam's mouth and holds him down like that. Sam doesn't know what to do with his hands when they're not tied behind his back, but he ends up holding on to Castiel's thighs where they're clenched over Sam's hips, and he finds it's like having an anchor.
