This is obviously all Dean's PTSD-fueled nightmare: Sam finding love with a girl in a pretty sundress and together they get a dog and frolic in the sunshine, picking flowers and snuggling. At the end of the episode Dean wakes up in the field, having been pushed out of Purgatory by Benicio del Toro, and he is lying on the ground, wet and naked.

Sam finds him and is like "DEAAAN!" and Dean croaks out "where's your pretty girl and your dog, you bastard??" and Sam cocks his head and shakes his pretty hair as the credits roll.
